Required documents


1. In order to assess insurance risk and provide services the applicant is required to:


1)  Fill in Insurance application form (Arm)

     Submit:

2)  State registration Certificate of the exporter and TIN (copy)

3)  Passport or identification card / copy / of the head of the exporting company or the individual entrepreneur

     If the application is signed by an authorized person, a passport or an identification card (copy) of the authorized person and a power of attorney

4)   Statement on the existence (absence) of the beneficial owner (in Armenian)

5)   Statement on affiliated persons (in Armenian)

6)   Export contract between exporter and buyer (if there is no signed contract – its draft), including the product specification, other applications and documents (Agreements) that are an integral part of the contract

7)   Documents confirming the delivery of goods to the buyer (if the supplies took place)

8)   Documents confirming the fulfillment of obligations by the buyer for payment of previously delivered goods, if earlier deliveries have been made and payments have been received

9)   Breakdown of accounts receivable and accounts payable in the form of reference (if required)

10)  Other documents if necessary


 2. In order to conclude an Insurance policy, the applicant is required to submit:


1)    Copy of invoice

2)    Customs declaration (if any)

3)    Copy of CMR transport document (if any)

4)    Breakdown of accounts receivable and accounts payable in the form of reference (if required)

5)    Specification (if any)

6)   Other documents if necessary
